What personal data we collect and why we collect it

Contact forms

Any information you provide by completing forms on our website or in our offerings sections (by requesting a free interview lesson, messaging us, or by getting in touch) will be collected, stored, and used solely for the purposes FMAA outlines below.

Sometimes the reason for collection will be obvious (such as when you give us your email address to allow us to contact you). When it is not, we will describe to you at the time of collection the purpose for collecting the data and if possible ask for your consent.

Caldera Forms stores a record of all form submissions. Your data may be deleted by the site administrator. You may request a report of saved data related to your email address.

FMAA forms store data which will remain on the server throughout the length of your enrollment with FMAA and for up to one year after.  You can unsubscribe anytime. Contact us at info@fellowshipmusicartsacademy.org to request to be removed from our database by the site administrator.

Some data may be shared with FMAA’s other third party services including PayPal, Teacher Zone, independent contractors (music instructors), and affiliate instructors of FMAA. FMMA may share your information in a referral with an affiliate instructor if applicable given your consent.

Collection of Personal Data From Children

FMAA’s website is not intended for or marketed to persons under the age of 18. FMAA does solicit personal information from children under the age of 13 and, without the written consent of the child’s parent or legal guardian, will not knowingly collect, post or distribute information from any child under the age of 13. If we learn that we have collected personal information from a child under age 13 without the written consent of that child’s parent or legal guardian, we will delete that information as quickly as possible. Parents of children under the age of 13 have the option of consenting to the collection and use of the child’s information without consenting to the disclosure of such information to third parties. In any event, we will not disclose such information to third parties unless they are under an obligation of confidentiality. If the parent or legal guardian consents, we may collect the child’s name, email address gender, date of birth, instruments played, and musical preferences directly from the child or his/her parent. We will use the information for purposes of tailoring curriculum to our students needs and interests, and for internal customer analysis/insights related work and will collect only such information as is reasonably necessary for those purposes. Consenting parents may, by sending an email to us at, info@fellowshipmusicartsacademy.org, review the information we have collected from his/her child, withdraw his/her consent to any further collection or use of the information, and ask that we delete the information we have collected.

Cookies

If you leave a comment on our site you may opt-in to saving your name, email address and website in cookies. These are for your convenience so that you do not have to fill in your details again when you leave another comment. These cookies will last for one year.

If you visit our login page, we will set a temporary cookie to determine if your browser accepts cookies. This cookie contains no personal data and is discarded when you close your browser.

We have integrated the component Google Analytics (with anonymisation function) on this website.

Our website also uses Google Analytics performance reports relating to demographics and interests and reports on Google Display Network impressions. Donations

If you provide donations (thank you!) through our donations page, we collection your contact information as with contact forms mentioned above.

Your donation collected on this site is made with PayPal, and your personal data will be transmitted to PayPal. If you have not yet opened a PayPal account, you will be asked to do so by PayPal in the course of the payment process. When you use or open a PayPal account, your name, address, telephone number and e-mail address must be transmitted to PayPal. The legal basis for the transmission of data is article 6, section 1 a, GDPR and article 6, section 1 b, GDPR.

Teacherzone

As a student of FMAA, we may require usage of the Teacherzone.com application. If you use this software, some data will be transferred between their system and you.

It is TeacherZone.com’s policy that personal information, such as your name, postal and e-mail address or telephone number, is private and confidential. Accordingly, the personal information you provide is stored in a secure location, is accessible only by designated staff, and is used only for the purposes for which you provide the information.

Media

If you upload images to the website, you should avoid uploading images with embedded location data (EXIF GPS) included. Visitors to the website can download and extract any location data from images on the website. FMAA may record audio and/or visual formats of student’s lessons, group classes, and/or performances. Any such recordings (photographs, video, sound recordings, etc.) may be subsequently used without compensation to the child or their family by FMAA, or third parties for publications (including websites), advertising, and/or publicity purposes at the discretion of FMAA. FMAA will not use students’ names or any other personal information online. If parents/guardians would like to opt-out of this media exposure, they can request and submit a written waiver.

Who we share your data with

Information obtained will be used for enrollment purposes only.  Some data upon enrollment with FMAA may be shared for use in conjunction with FMAA’s other services including PayPal, TeacherZone, or with FMAA independent contractors and affiliates. 

How long we retain your data

If you leave a comment, the comment and its metadata are retained indefinitely. This is so we can recognize and approve any follow-up comments automatically instead of holding them in a moderation queue.

For users that register on our website (if any), we also store the personal information they provide in their user profile. All users can see, edit, or delete their personal information at any time (except they cannot change their username). Website administrators can also see and edit that information.

What rights you have over your data

If you have an account on this site, or have left comments, you can request to receive an exported file of the personal data we hold about you, including any data you have provided to us. You can also request that we erase any personal data we hold about you. This does not include any data we are obliged to keep for administrative, legal, or security purposes.

Opt-Out policy

Visitors may opt-out of having their personal information collected by FMAA, used by FMAA for secondary purposes, disclosed to third parties or used by FMAA or third parties to send promotional correspondences to the visitor, by contacting FMAA and filling out a waiver at info@fellowshipmusicartsacademy.org

Correcting personal data

Upon request via postal mail, e-mail, or phone, FMAA will provide to visitors a summary of any personal information retained by FMAA regarding the visitor. Visitors may modify, correct, change, or update their personal record or cause their personal record to be removed from FMAA’s database. Visitors may do this of their own accord by logging into Teacher Zone (the online learning platform FMAA uses). FMAA will only send personal records to the e-mail address on file for the visitor name associated with it.
